As the school year comes to an end, let me wish you all a happy and healthy summer break. Though we are also looking forward to the summer break, we are already planning for the 2013-14 school year.
If you have a child who will be in kindergarten next year, you will need to provide a few pieces of documentation. Your child will need to have had a physical examination by his/her doctor within a year of the first day of school. The doctor will need to fill out the Physician Report form. You will also need to provide a current immunization record. Along with this, a parent will need to fill out a Health History form. Physicals and immunization records need to be turned in no later than 2 weeks after the first day of school. Students without the proper documentation by this deadline will be excluded from school until such a time when the necessary paperwork is received.
If you have a child who will be in 7th grade, he/she will need to have a Tdap booster. This immunization is typically given between the ages of 10 and 12. It is necessary to turn in documentation of this immunization within 2 weeks after the first day of school. Students without the proper documentation by this deadline will be excluded from school until such time as the proper paperwork is received.
Finally, several options are available to help with these requirements. The Hamilton County Public Health Department holds immunization clinics in Reading. Their representatives will be at the municipal building the 3rd Wednesday of each month from 12:30-3:30. You will need a copy of your child’s immunization record. There is an administrative fee of $15, but the immunizations are free. Also, The Health Care Connection is offering sports and back to school physicals and dental check ups for children who may not already have a doctor or a dentist. These physicals and dental check ups will be on July 27, 2013 and August 17th 2013 at the Lincoln Heights HealthCare Center. They accept Medicaid, Medicare, and many traditional insurance plans, as well as self pay patients.
