Dear Parents/Guardians,
Students entering the 7th grade for the 2010-2011 school year are now required by the Ohio Dept. of Health to receive a booster dose of Tdap or TD vaccine PRIOR to entering the 7th grade. This affects our current 6th grade students for next year.
This vaccine is for diphtheria, tetanus (lockjaw), and pertussis (whooping cough).
Proof of this vaccine will be required prior to entrance into the 7th grade next school year. Please make sure you obtain this vaccine over the summer and bring the paperwork to school by the first day of school or your child will not be permitted to start school. We will be sending reminders per our school newsletters and on the school website. Please see the link for the list of Hamilton County immunization clinics and directions.

PLEASE NOTE: Hamilton County Public Health upcoming vaccination clinics for students who will be entering 7th grade. Students entering 7th grade who need the required dose of Tdap (Tetanus, diphtheria and acellular pertussis) may receive it free of charge at the following locations:
May 1: Taylor High School
36 Harrison Ave., North Bend, 45052
10 a.m. – 2 p.m.
May 8: R.G. Cribbet Recreation Hall
5903 Hawthorne Ave. Fairfax, 45227
10 a.m. – 2 p.m.
Visit www.hamiltoncountyhealth.org to access the Consent Form, Vaccine Information Statement and the Notice of Privacy. For efficiency purposes parents may wish to print and complete the Consent Form prior to attending the vaccination clinic.
Tdap vaccine will also be available at HCPH’s regular immunization clinics located at 13 sites across the county. Please visitwww.hamiltoncountyhealth.org and choose Immunization Clinic Locations from the Quick Links drop down menu at the top of the page to view clinic dates and times. If you do not have access to a computer, clinic locations can be found by calling 513-946-7949. Additionally, parents may choose to have their child vaccinated at their primary care physician’s office. Please contact your physician to determine vaccine availability.
