 Wanted to let everyone know that we will be offering sports physicals
once again this year. They will be held on July 22 begining at
6:00p.m and ending at 8:00p.m.. The physicals will be held at the Reading Family Practice
office. The address for the office is 9400 Reading Road. The cost of
the physical will be $15.00 and a OHSAA physical form is needed at the
time of the physical. The first page of the form needs to be filled
out and signed by the parent or guardian. |
