Preschool & Kindergarten Requirements

Preschool and Kindergarten health documentation requirements:

In order for your child to attend Preschool or Kindergarten at Reading Elementary School, the following health documentation will be required:

-Most current copy of immunization record

-Physical examination completed within the last year 

-Ohio Health History 

-Dentist report (optional, but helpful)

Your child must be up to date on their immunizations according to the CDC childhood immunization schedule (linked below) or have a valid exemption on file. Please ensure the school nurse receives copies of any other medical documentation related to any pertinent medical history, diagnoses, allergies, or other information that we should be aware of. Please see the Immunizations tab for more information regarding immunizations. 

Any prescription medications that need to be administered at school will require a prescription medication form to be completed. The medication should be brought into the office by a parent or guardian in the original packaging. 

You may complete permissions for some over the counter medications in Final Forms. If there is an over the counter not listed that you would like to have administered at school, there is an over the counter form that can be completed.
